Remote Health Monitoring
We keep an eye on your health between visits — automatically. We provide you with a simple device (like a blood pressure cuff or glucose monitor) that sends your readings directly to our care team. If anything looks concerning, we reach out before it becomes a problem.
How it works in plain terms: You take your readings at home. The device sends the numbers to us. We review them daily and call you if anything needs attention. No tech knowledge required.
Best for: High blood pressure, diabetes, heart conditions, COPD, or anyone recently discharged from the hospital.

Chronic Care Management
Living with a long-term condition like diabetes, heart disease, or high blood pressure means your health needs attention every day — not just when you come in for a visit. Our Chronic Disease Management program gives you a dedicated care team that checks in monthly, helps coordinate your care, and makes sure nothing falls through the cracks.

Circulation Testing (ABI)
Poor circulation in the legs is surprisingly common — and often completely silent until it becomes serious. Our Circulation Test (called an ABI, or Ankle-Brachial Index test) is a quick, painless in-office test that checks how well blood is flowing to your lower limbs. It takes about 15 minutes and can detect a dangerous condition called Peripheral Artery Disease (PAD) before symptoms appear.
In plain terms: We take blood pressure at your arm and your ankle and compare the two numbers. A significant difference signals that an artery may be narrowed — something we can manage if caught early.
You may need this if: You're over 65, have diabetes, smoke or smoked, or have high blood pressure or cholesterol.
